Alcatel OT-5020N unlock problem

hi Furious team

today I received Alcatel one touch OT-5020N with The slot has been permanetly locked problem...

I used last OTsmart version and I got the codes but phone doesn't show any bar or any option to put the codes(even I pressed *#*#3646633#*#* and go to simme lock but all options are disabled)
